Within 30 days of the activation of a General Assignment, ISG will mail a Creditor Bulletin and Proof of Claim form, along with Form W-9, to all known case creditors, as provided to ISG by the debtor company. Creditors may confirm their mailing address with the Assignee’s Claims Agent at (949) 494-7160.
Creditors are advised that in order to participate in any possible distributions of funds to creditors, they must file a Proof of Claim with the Assignee’s Claims Agent in accordance with the instructions provided on the Proof of Claim form. Please note that, due to recent changes in the I.R.S. reporting requirements, a completed Form W-9 must accompany every submitted Proof of Claim.
ISG requests that backup documentation accompany the submitted Proof of Claim to support the claim amount.

Creditors seeking confirmation of receipt of their Proof of Claim should include a self-addressed stamped envelope and request for confirmation of receipt, along with the mailed submission of their Proof of Claim.
