Claims Submission

Within 30 days of the acceptance of a General Assignment, Insolvency Services Group will mail a Creditor Bulletin and Proof of Claim form, along with Form W-9, to all known case creditors, as provided to ISG by the debtor company.

Creditors are advised that in order to participate in distributions to creditors, they must file a Proof of Claim with the Assignee’s Claims Department in accordance with the instructions provided on the Proof of Claim form.

ISG requests that you include backup documentation with your Proof of Claim in support the amount claimed. The Proof of Claim may be submitted by mail, fax, or by email to:

Creditors seeking confirmation of receipt of their Proof of Claim should include a self-addressed stamped envelope and request for confirmation of receipt, along with their mailed Proof of Claim. Creditors may also call the Assignee’s Claims Agent at (949) 494-7160 to confirm ISG’s receipt of their Proof of Claim.
